Chicago Family Case
Management, funded by DHS, started as Families with a Future, later
becoming Healthy Moms/Healthy Kids. CFCM provides assistance to
medical care for over 4,000 pregnant women and infants up to one
year of age. Services include home assessment, bus fare for WIC, and
medical appointments. Other services include appropriate referrals.
CFCM also holds an annual community baby shower for approximately
60-100 pregnant women and new moms.
